Certified Home Health Agency

The certified home health agency (CHHA) provides intermittent care through home visits for patients with acute and chronic conditions. Services assist the patient in recovering and/or enabling families to maintain the patient at home.

¨ Assessment, wound care, IV therapy, catheter care, phlebotomy, tracheotomy care, med pours, disease management, and health care guidance are offered by skilled nurses.

¨ An Occupational Therapist is also available to work with patients in a home care setting.

¨ Physical therapists offer home exercise programs, and instruction of use of assistive equipment.

¨ Speech therapy is available for evaluations, exercises, and guidance.

¨ The medical Social Worker assists patients and families with problems affecting recovery.

¨ Home Health Aide and services are available to assist with activities of daily living and light housekeeping.

Visit frequency is dependent upon acuity and can range between twice daily to once monthly. A registered professional nurse is available 24 hours per day to handle “after hour” situations.

Referrals are accepted from hospitals, nursing homes, physicians, community agencies, family, friends, or the patient.

Medicare, Medicaid, and third party insurances are accepted and a sliding fee scale is utilized for the “un” or “under” insured patients. Grant monies from the Office for the Aging assist with providing aide services when eligible.

No one is denied service due to their inability to pay.

The home care team works with the physician to develop a plan of care specific to the patients' needs.

Available Services (Detailed):

Skilled Nursing visits are available to patients with cardiac, respiratory, endocrine, musculoskeletal or neurological conditions. Visits can be done for medication instruction and weekly set up, dressing changes, blood draws, catheter changes, ostomy changes, IV med infusion, injections, diabetic care and teaching, tube feedings and monitoring vital signs.

Physical Therapy visits are available for assessment of patients with acute musculoskeletal problems. Therapists are responsible for safety evaluations of the home, development of a home exercise program and educating the patient/care giver on use of assistive devices when necessary.

Home Health Aide visits are offered to assist the patient with bathing, dressing, toileting, ambulation, meal prep and light housekeeping.
